Before Your First-time Visit:
If you have made an appointment for a private lactation consultation, you will need to fill out a Consent form and a Maternal & Infant Information form before you can be seen. The lactation consultant will need to see your baby nurse and keeping
a hungry baby waiting while mom fills out paperwork can be stressful. In order to avoid this situation, we suggest having these forms already filled out when you come in. This will ensure that your First-time Visit can start with as little delay as possible.
Payment for services are due at the time of service. You may pay by cash, check, Visa, Mastercard or Dicover cards.
